Please use this form to request a copy of your Baptismal Record with Notations if you were Baptized at St. Mark the Evangelist Catholic Church, or if you need a copy of a Sacrament received here. We will need a copy of your driver's license to release the record.
Parish staff who are preparing members for Sacraments may also use this form to request records, and they will be sent directly to your parish.
We will respond to your request within seven (7) business days from the date of receipt.
This form may only be used by adults aged 18 years or older requesting: 1.) Their own Sacramental record, or 2.) Their minor-aged child's Sacramental record.
